Medical Records will be available for pick up. If immunization records are needed, please access your <a href=\"https:\/\/ciis.state.co.us\/public\/Application\/PublicPortal\" target=\"_blank\"><strong>Colorado Immunization Information System (CIIS) portal here<\/strong><\/a>. The CIIS produces an official record of immunization and is accepted for schools, child care, travel, etc.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong><a href=\"\/wp-content\/uploads\/sites\/4\/2026\/05\/CMM_Authorization_for_the_Release_of_Health_Information_June2024.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Medical Records Release Form &#8211; English<\/a><\/strong><\/li>\n\n\n\n<li><a href=\"\/wp-content\/uploads\/sites\/4\/2026\/05\/CMM_Authorization_for_the_Release_of_Health_Information_June2024_SP.pdf\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Autorizaci\u00f3n para divulgar informaci\u00f3n de salud del paciente<\/strong><\/a><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Submit medical records release forms via fax to (970) 926-6348.
