Diabetes Self-Management Education Program
This program is designed to help patients manage their diabetes more effectively through a variety of tactics such as:

• Nutrition
• Exercise
• Behavioral Health
• Medication
• Reducing diabetes related complications

This program is designed for patients that have been diagnosed with diabetes or already have diabetes. Patients need a referral from their provider or you can call (970) 363-5428 to find out if you qualify. The Diabetes Self-Management program is covered by most insurance.
