The CMM Clinical Center’s FollowMyHealth® Patient Portal is an internet-accessible application that offers patients secure access to pieces of their Colorado Mountain Medical electronic medical record.
Patients will be able to access pieces of your electronic medical record, which includes discharge summaries, discharge instructions, outpatient first registration reports, inpatient history and physical examination notes, rehabilitation notes, procedure notes and some laboratory, cardiology, and neurology results. If you cannot locate specific information, please message your provider first.
  • Through your internet browser go to the Colorado Mountain Medical’s FollowMyHealth® Patient Portal page https://www.followmyhealth.com/
  • Once there, you will need to create a new account by clicking on the “I Need to Sign Up” box.
If you already have a FollowMyHealth® account:
  • Select My Account in the top toolbar and select My Connections.
  • In the With Providers and Healthcare Organizations tab, click Add Organization.
  • Perform a search of the zip code "81620" and you will see "Colorado Mountain Medical" listed as an available organization. Our Avon location will show up as the primary location as it is our headquarters.
If you do not have a FollowMyHealth® account:
  • Create an account following the steps listed above.
  • After you have successfully created your account, you will be asked if you would like to request a connection to an organization.
  • At this time, perform a search of the zip code "81620" and you will see "Colorado Mountain Medical" listed as an available organization. Our Avon location will show up as the primary location as it is our headquarters.
  • After you request a connection to Colorado Mountain Medical please allow one to two business days for your connection to be reviewed and established. If you experience any problems please contact our Patient Portal Support team. This team is available Monday through Friday 6am-6pm MST (excluding federal holidays) by phone (888) 670-9775.
Most often, your user name is your email address that the CMM team has on file.
Yes, you can create your own password and change it when necessary. Password requirements are: at least 8 characters, must contain at least one number, and one special character.
Look for the “Forget your password?” link under the user name and password field on the Sign-In page. You will be prompted to enter your Colorado Mountain Medical FollowMyHealth® Patient Portal account username. Once you have entered this information, a message will be sent to your email address providing you with a link to reset your password.
Yes. There are two ways to set up a proxy account in FollowMyHealth®, the patient can request a proxy account in your account or someone can request a proxy account on your behalf. To add a proxy account please call (888) 670-9775.
Yes. FollowMyHealth® Patient Portal can be downloaded through the Apple and Android app stores.

Yes. FollowMyHealth® Patient Portal is a secure internet site which is protected through advanced encryption technology. Access to your account is secured through your own personalized user name and password, known only to you. It is important that you keep this information in a secure place and do not share it with others.
As long as your provider is not on vacation, messages will be addressed within two business days (Mon – Fri).  Providers only check the messages when they are in the office.
No. Non-CMM healthcare providers cannot access your electronic medical record through the CMM FollowMyHealth® Patient Portal. If you have another provider who needs copies from your medical record, please fill out the form or contact the Health Information Management at (970) 926-6340, ext. 2230.

Authorized test results will be available for patients to view within 24 hours of the result being finalized by your doctor. Documents and reports are available in the FollowMyHealth® Patient Portal once they are signed and completed by your physician.

Yes. You will be able to message your listed Attending and Primary Clinicians, in addition to any staff member that has requested to message you through the system. Messages will be answered within 2 business days, depending on when the provider is in the office.  If you do not see a staff members name in your messaging option list, please contact the Health Information Management at (970) 926-6340, ext. 2230.

You cannot update your email address within the Colorado Mountain Medical FollowMyHealth® Patient Portal. You may request updates to any demographic information, including your email address, when you visit the CMM. If you need to request an update at any other time, please contact the Patient Portal Support Team Monday through Friday 6 AM to 6 PM MST (excluding federal holidays) by calling (888) 670-9775 or sending an email PatientSupport@FollowMyHealth.com.
No. The Colorado Mountain Medical FollowMyHealth® Patient Portal is provided as a courtesy to our patients and the use is optional.
No. The Colorado Mountain Medical FollowMyHealth® Patient Portal is not to be used for emergency related healthcare issues. Please contact your physician directly for any questions or concerns. For medical emergencies or immediate care, please call 911.
You have a few options to view your Colorado Mountain Medical upcoming and past appointments:

On your phone: To view appointment information, click on the Appointments icon at the bottom of the screen. You will have the ability to view upcoming and past appointment information.

On a desktop computer: Appointment information will display on the right side of the Home screen as soon as you log in.
At this time, you are only able to view appointments. For any changes or to make an appointment please contact a member of your research team.
